Snell & Wilmer Represents Blueprint Equity in Growth Investment in Careflow

SAN DIEGO – Snell & Wilmer represented San Diego-based client Blueprint Equity, a technology-focused growth equity firm, in its investment in Careflow’s growth financing. Careflow is a CRM and referral intake platform designed to streamline administrative and financial workflows across the long-term post-acute care (“LTPAC”) sector.

Careflow addresses inefficiencies in referral management by consolidating patient and facility data into a unified system of record and automating workflows such as referral intake, admissions, case management, and contract management. The platform currently supports more than 500 facilities across skilled nursing and assisted living settings, with growing adoption among home health agencies and hospice providers.
